International providers are under no obligation to file claims on behalf of our members. You may need to pay for the services at the time you receive them and then submit a claim to us for reimbursement. Claim forms are available at www.bcbsfepvision.com or via email at fepmemberhelp@davisvision.com. To file a claim for covered vision care services received outside the United States and Puerto Rico, send completed claim forms and itemized bills to:
